57% of enterprises have watched AI agents be confidently wrong. The fix is an agentic context layer, but who has one?

A VentureBeat survey of 101 enterprises found that 57% traced confident but incorrect AI agent responses to missing or inconsistent business context, with 31% seeing it happen more than once. Most companies still rely on document retrieval for agent context, while only 25% have a governed agentic context layer in production and 34% are building one.

The findings suggest enterprise AI failures are shifting from model capability issues to data governance and context infrastructure gaps. As agents move deeper into business workflows, demand should grow for context platforms that standardize definitions, metrics, permissions, and retrieval quality across tools.
